AGREEMENTS re: Ranking for Master Teacher Position

Agreements:
1.      Performance Rating requirements-VS in the PAST (8.56 onward)
2.      The maximum points shall be given when required documents are available or no point when the required documents are not satisfied
3.      Public Schools District Supervisor shall be required to submit a District Memo announcing the vacancy to be deliberated.
4.      Documents of all candidates with transmittal to be submitted to the Area Supervisor one week before deliberation.
5.      Candidates must submit three copies of the documents (1 original copy and 2 photo copy)

DOCUMENTS TO BE SUBMITTED BY THE CANDIDATES:

A.     Basic Requirements
1.      Service Records (prepared by the Records Section and signed by the OA)
2.      Latest appointment
3.      Performance Rating for the last 2 years(prepared by the service records)
4.      Class/Intermediate Program for the last 2 years
5.      Certification under oath of the school head that the candidate is actually teaching
6.      Transcript of Records with SO or RN
7.      PRC license or certification of good standing issued by the said agency

B.      Leadership Potentials and Accomplishment:
All claims for all criteria should be supported by a certification under oath signed by the school head/supervisor and corroborated by 5 other personnel involved in the activity.

Creteria A – Introduced any of the following which has been adopted or used by the school/district/division

1.      Claim for Curriculum or Instructional Materials
·         Approved Proposal and write up signed by ASDS
·         Copy of the curriculum Materials
·         Result of validation and copy of the validation instruments used.
·         Certification of School heads/teachers who adopted the materials

2.      Claim for Effective Teaching Techniques or strategies
·         Approval action research proposal and write up signed by the ASDS
·         Copy of the lesson plan
·         Result of validation and copy of the validation instrument used.
·         Certification of School heads/teachers who adopted the materials
3.      Income Generating Projects
·         Approved proposal and write up signed by the ASDS
·         Certification/other documents showing where income generated was used/Financial statement signed by the principal
·         Certification of the school head/teachers who adopted the materials
·         Certification for being the proponent of an income generating project for 2 consecutive years, corroborated by 5 teachers and duly notarized.

4.      Simplification of Work for Record Keeping
·         Certification corroborated by 5 teachers and duly notarized
·         Approved proposal and write up signed by the ASDS
·         Work plan
·         School Memo
·         Narrative report

Criterion B- Served as subject coordinator or Grade Chairman for at least 2 years

1.      Claim as Subject Coordinator or Grade Chairman for at least 2 years
·         Certification for serving as subject coordinator and satisfactorily discharged such assignment for 2 years, provided such assignment or services are in addition to and not considered part of the regular load, corroborated by at least 5 teachers and duly notarized

·         Designation as coordinator signed by the PSDS for District Coordinatorship and School Heads for School Coordinatorship

·         Accomplishment report signed by the PSDS for district coordinatorship and school head for school coordinatorship

·         Action plan

2.      Adviser of Interests Clubs
·         Designation signed by SH or PSDS
·         CLASS/Intermediate program with teaching load on the SY being claimed
·         Action Plan approved by SH/PSDS
·         Accomplishment Report attested by the principal

3.      Adviser of school publication
·         Designation signed by the SH/PSDS
·         Class/Intermediate program with teaching load on the S/Y being claimed
·         Action Plan approved by SH/PSDS
·         2 Cpies of publication for the 2 years being claimed





              Criterion C- Served as chairman of a special committee

1.      Credited for curriculum/instructional materials are: modules, guide, manuals, lesson plan, worksheet, exercises, workbooks

2.      Credited for school program are : testing, reading, mathematics, remedial and enrichment programs

Documents:

·         Memorandum on the organization of the special committee signed by the SH/PSDS
·         Certification for serving as chairman corroborated by 5 teachers and duly notarized
·         Project proposal signed by SH/PSDS
·         Action Plan signed by the SH/PSDS
·         Copy of the outputs curriculum/instructional materials or prepared program
·         Write up of accomplishment signed by SH/PSDS

                    Criterion D- Initiated or heade or participated in an educational research
·         Letter request to the ASDS to conduct an action research
·         Approved research proposal and write up/research report signed by ASDS (for participation as member 7 points)


                   Criterion E- Coordinator of community project or activity or of a program
                   Of another agency or coordinator of rural service improvement activity
                   (feeding, nutrition, agro-industrial fair for at least 2 years)
·         Designation signed by SH/PSDS
·         Certification as coordinator of the project for at least 2 years signed by the barangay chairman/officials
·         Action Plan approved by SH/other agencies
·         District/School Memo/Programs where candidates is named as coordinator
·         Report of accomplishment attested by SH or PSDS same s/y with the action plan
·         Sample evidences of accomplishment as indicated in the action plan and report of accomplishment for the s/y upon which the candidate is claiming
·         Awards of recognition/appreciation signed by sponsoring agency
              

                   Criterion F- Organized or managed an in service training activity or other
                   similar activity at least in the school level (coordinator who organized or
                   managed INSET shall not be given points under this criterion, it is an inhe
                   rent function as a coordinatorship

·         Memo where the candidate is designated as over all chairman/organizer/manager of the INSET
·         Certification for organizing/managing the INSET, corroborated by 5 teachers and duly notarized
·         INSET Proposal with training design approved by the PSDS, If district level or school head for school level
·         INSET report of accomplishment signed  by the candidate and School Head  for school level and PSDS for district
·         Certification of Merit/Commendation

               
                Criterion G- Credited with meritorious achievement such as 1) trainer for
                Contestant who received prizes in the National, Regional or Division level
                Or  2) athletic coach of athletes who received prizes (this must be either a
                DepEd sponsored or endorsed activity)

·      Memorandum cited as winning coach
·      Certificate of award as winning coach


                Criterion  H- authorship
·      Copy of the book or articles where the name of the authors in it.



                Criterion I – Demo teaching
·      Approved lesson plan signed by PSDS/ES
·      MEMO/TIP/PROGRAM of activities where the candidate is cited as demo teacher
·    Certification stating the exact date when the candidate served as demo teacher and corroborated by at least 5 teachers and duly notarized
